Initial/Current Assessment

  • The Initial Assessment is to be completed shortly after a service user is admitted, it should be used to capture the service user’s needs at that point in time, once completed it’s set in stone so it can be used as a reference point
  • The Current Assessment should be exactly that, current, it allows for all the service user’s needs to be in one place
  • Each need can later in the care planning process be linked to the relevant aspect or aspects of life in the care plan
  • Blue symbol provides relevant informs from assessment tools
  • Yellow symbol shows previous entries, full detailed history for each section
  • Green tick identifies the number of care plans where this risk is managed
  • Faint grey text below details ADLs where this risk is managed
