Care Plans are split into ADLs, or Care Plan Needs
Each Care Plan Need contains
Assessed current situations – Allows for the linking of relevant sections from the current assessment and assessment tools to this Care Plan Need
Care needs – Allows the care planner to summarise the Assessed current situations, or add additional information
Add self-sufficiency – a section for entering what the service user is able to do for themselves
Outcome/goal – What is the service user looking to achieve
Description of care actions – Clear, succinct instructions on how to support the resident to achieve the outcomes or goals
Actions to track for reviews – Shows actions related to the Care Plan Need or ADL, clicking on one shows all notes against that icon for that resident since last review, allowing for real feedback from frontline care when reviewing care plans
