Who I am
- “Who I am” - Collates information from various sections into a personal profile. Additional fields with user prompts to promote the capture of resident’s personal profile information in a consistent and comprehensive manner
- Care App / Mobile Devices – new Who I am profile, including
- Important to me – routines, what is important to me, likes to talk about, things that may worry or upset me & what makes me feel better if I am anxious or upset
- How best to support me – the actions from care needs
- My Life Story – what people like and admire about me, important people and places, childhood memories, working life, later life, interests and hobbies, and significant events
- Wishes for the future – key information about thinking ahead, concerns, symptom management and after death. This section will show first (i.e. before all the other sections) for residents at the end of life stage.
- Care need actions show those that have been updated in the last 2 weeks to clearly show anything that may have changed recently regarding how best to support an individual’s needs

Best use of MCM
Who I Am is compiled from sections of the care plan, current assessment and service users details. You can view which sections pull through to the Who I Am page by selecting 'Who I Am' at the top of the service user's profile page on Monitor. Do your staff know it's also on their devices? Tap the resident's photo 4 times!
