- Information from assessment tools is used to inform the whole care planning process
- Services can set mandatory assessments in line with their policies, mandatory assessments have a grey arrow next to them
- Assessment tools, as with paper, mostly consist of multiple-choice questions
- Some assessment tools give a score, those that do will also inform you of what that score means

Best use of MCM
If an assessment score suggests a medium to high risk, 'please link to a care plan need' will appear in red. This indicates the risk has not yet been linked to a care plan need, meaning the risk is not being managed. Assessments can be linked to care needs within the long term care plan.
